STATE MEDICAL ORGANIZATIONS AS SUBJECTS OF THE MEDICAL SERVICES MARKET UNDER COMPULSORY HEALTH INSURANCE

Abstract.

The article examines the transformation of the position of state medical organizations in the context of the market model of healthcare and the compulsory health insurance (CHI) system. The institutional duality of state institutions forced to combine the implementation of state assignments with the need to ensure economic efficiency and competitiveness in the presence of private medical organizations in the CHI market is analyzed. Based on the analysis of the evolution of the Russian CHI model, regulatory changes in 2025–2026 and empirical data, key contradictions in the financial support of state medical organizations, personnel imbalances and administrative barriers are identified. Directions for improving the organizational and economic mechanism of functioning of state medical organizations as full-fledged subjects of the medical services market are proposed.
